Role Overview:

As a Patient Transition Liaison, you will play a crucial role in supporting DischargeHUB by managing service requests, maintaining communication with stakeholders, and ensuring smooth transitions for patients from hospital to home. This role involves both administrative duties and direct interaction with healthcare providers, patients, and their families.

Key Responsibilities:

Service Request Management:

Generate service requests via phone and email promptly based on incoming requirements.
Follow up on service requests to ensure they are addressed in a timely and efficient manner.
Liaise with agencies, vendors, and NLHS discharge facilitators to ensure service requests are prioritized and resolved.

Communication:

Maintain open lines of communication via phone and email with stakeholders to keep them informed of the status of their service requests.
Address any concerns or issues raised by stakeholders in relation to service requests.

Administrative Duties:

Maintain and update records of service requests, ensuring all data is accurate and up-to-date.
Coordinate with vendors, agencies, and clinical staff to streamline operations and enhance efficiency.
Participate in regular team meetings to discuss operational challenges and brainstorm solutions.

Continuous Improvement:

Provide feedback on the service request process and suggest improvements.
Stay updated with the latest industry best practices to bring innovative ideas to the team.

Procedural Documentation:

Document and maintain standardized operating procedures (SOPs) for all relevant tasks and processes within the operations department.
Ensure SOPs are accessible and understandable for all team members.
Regularly review and update SOPs to reflect any changes in processes or best practices.
Collaborate to ensure consistency and efficiency across all procedures.

Qualifications:

Diploma or Bachelor’s degree in Healthcare Administration, Business, or a related field.
Experience in service request management, customer service, or healthcare administration.
Strong interpersonal and communication skills, with the ability to build and maintain relationships with diverse stakeholders.
Excellent problem-solving skills and a proactive approach to addressing client needs.
Ability to analyze data and derive insights to inform decision-making.
Familiarity with healthcare industry regulations and best practices is a plus.

Employment Terms:

Full-Time Role:

Hours: Monday through Friday, with occasional evening and weekend shifts.
Part-Time Role:

Hours: Flexible, coordinated with supervisor.

Additional Information:

Location: Remote work environment and Seafair office (St. John’s).
